Rock Your Way to Wellness Fitness Challenges ~ May 7 – June 15, 2012

Get those “blue suede shoes” polished up and get ready to Rock ‘n Roll your way to wellness!

The Rock Your Way to Wellness individual and team challenges are a rockin’ fun way to achieve your physical activity goals!  Start off at the Rock ‘n Roll Hall of Fame in Cleveland, Ohio and travel all over the world learning about some of the most well know Rock ‘n Roll Hall of Fame inductees.  Also, keep a look out for the Rock ‘n Roll Trivia milestones as you track your progress during this 6 week challenge!

  • Join as an individual and track your daily physical activity.
  • Create or join a team of 3 – 5 and track your daily physical activity.
  • Reach designated milestones along the way to be entered into drawings for USB pedometers.
  • Average 10,000 steps/day to complete the challenge by June 15th and be entered into a grand prize drawing for a healthy team lunch or individual gift card!

Brickyard 400 TEAM Weight Loss Challenge ~ May 7 – July 27, 2012

Shed pounds as you make your way to the Brickyard 400 in Indianapolis, IN in time to cheer on your favorite driver!

  • Create or join a team of 3 – 5 and update your weight weekly
  • Achieve team weight loss milestones to be entered into pedometer and frozen yogurt/smoothie drawings.
  • Maximum of 16% team weight loss for the challenge.
  • Achieve 6% weight loss as a team to be entered into grand prize drawing for a $100 healthy team dinner.

KHC Provider Consolidated Measurement Reports

More than 1,000 clinicians in Greater Louisville received their 6th annual Consolidated Measurement report on the quality of care their patients received in the following clinical areas of focus:  1) diabetes, 2) breast cancer screening, 3) acute low back pain, 4) upper respiratory infection, 5) pharyngitis, and 6) bronchitis.  This report allows physicians and nurse practitioners to compare their own data to the community average and benchmark.

To implement this initiative, Anthem Blue Cross Blue Shield, Humana Health Plan of Kentucky, and Passport Health Plan provided claims data derived from their 2011 HEDIS submissions;University ofKentucky consolidated these data.  The following clinical areas are ONLY aggregated at the community level and not at the individual provider level:  1) cervical cancer screening, 2) chlamydia screening, 3) colon cancer screening, 5) depression, and 6) mental illness hospitalization follow-up.
