Positive Deviance: How Communities Can Rapidly Transform Diabetes Care

Attendees will learn about the current state of diabetes in our community, gain insights from regional benchmark providers who reached levels of optimal care for their patients with diabetes, and hear the story of one patient who has overcome diabetes.
